Natural-looking fat transfer with Dr Kian

Fat transfer (also called fat grafting, lipofilling or lipomodelling) is a procedure where autologous (your own) fat is taken from one area of your body and transferred to another area of your body that is lacking volume or needing more definition. This is a popular treatment to improve the contours of the face and breasts.

For the breast, fat transfer is an increasingly popular technique that enhances the size of your breast or without the need for prosthetic or silicone implants. This technique is suitable for people who require modest breast enlargement and have excess fat elsewhere.

For the face, this procedure can also be done to enhance definition with natural-looking results. This can be done in areas where you naturally lack volume in or have lost volume in due to ageing. It can also be used to treat fine lines and wrinkles. This procedure is similar to dermal filler treatment, however fat transfer is more long-lasting compared with hyaluronic acid fillers which degrade over time. Fat transfer can be carried out alongside other facial or neck rejuvenation procedure. Besides volume enhancement, there has been increasing interest and research in the anti-ageing and regenerative potential of autologous fat, as we now know that it is the richest source of mesenchymal stem cells in the body.

Most people who seek fat transfer would like to add volume to the breast or enhance certain features of their face. Using your own fat has the benefit of removing fat from a part of the body where it is undesired and using it to enhance another part of the body without the use of implants while still giving long-lasting results.

As we age, our faces naturally lose volume and appear more deflated due to changes in our skin, fat, muscles and bones. Our skin loses elasticity and becomes more saggy, while years of muscle use cause wrinkles to form. We lose volume in our fat and bones, resulting in a deflated or sunken look. Traditional facelift surgery does not address all these changes and volume enhancement can be provided with fat transfer.

Fat transfer can be used in the breast for volume enhancement or volume replacement. Volume replacement is increasingly popular for patients who previously have had breast implants and would like to have them removed. Fat transfer provides an option to replace the lost volume. Fat transfer can also be used to improve the shape and fullness in certain areas such as the upper breast and cleavage. It can also be a good procedure to correct contour irregularities such as following breast excision (lumpectomy) surgery.

  1. Am I suitable for fat transfer?

    Fat transfer is suitable for both male and female patients. It is a popular treatment for facial rejuvenation as well as breast volume enhancement (enlargement).

    For facial rejuvenation, fat transfer can be used to treat age-related volume loss, enhance or improve definition of your facial features. For facial rejuvenation, only a small amount of fat is required to be taken from other areas of the body and there is usually enough available in slim patients.

    If you are seeking fat transfer to the breast either for volume enhancement or replacement (replacing a removed implant with fat), you would usually require more fat available to be taken (usually from your abdomen or flanks).

    If you are over 18, generally healthy, not overweight and a non-smoker, you are suitable for fat transfer surgery. Occasionally, some people who seek fat transfer may actually not be suitable, and may require a different treatment such as non-surgical rejuvenation procedures (such as anti-wrinkle injections and dermal fillers) or breast enlargement with implants. During your consultation, you will be advised on whether autologous fat transfer is suitable for you.

  2. What will the consultation involve?

    At your consultation, you will meet Dr Kian, a fully-qualified plastic surgeon who is trained in the full range of fat transfer techniques. He will take a full medical history including details of medications, allergies and smoking. Dr Kian also always takes time to understand what your goals and concerns are.

    During your examination, the following will be assessed systematically and standard clinical photographs will be taken for your medical record:

    • areas to be treated
    • facial fine lines & wrinkles, skin quality (for facial fat transfer)
    • breast & chest wall measurements (for fat transfer to the breast)
    • asymmetry
    • possible areas where excess fat can be taken

    Dr Kian will then discuss your procedure with you. He will explain the potential risks and complications of surgery so that you make a fully-informed decision. There will be plenty of time to answer any questions that you may have and you can rest assured that Dr Kian will give you his honest, professional advice.

  3. What sort of scars can I expect?

    Small cannulas are used to harvest and inject fat through small incisions. Although incisions are small and placed discreetly, there may be some visible scars. Dr Kian will discuss what you are likely to expect at your consultation.

  4. When will I be able to see my results?

    Fat transfer will produce immediate results however you will have post-surgical swelling and bruising that will take up to 2 weeks to subside. Sometimes, bruising can take several weeks to completely disappear. Full results can take up to 6 months, however if you put on or lose weight in the future it will have an impact on your results. Some of the fat will resorb over time, however the amount of resorption will vary between individuals. Scars may continue to be red initially but will usually fade well with time.

  5. Will I be able to wear make up after fat transfer to the face?

    You will be able to wear make up once all the incision wounds have healed, and this is usually a week after your surgery. It is not recommended to apply make up over the incision sites.

  6. What are the risks/complications?

    Most patients are very satisfied with the results of their fat transfer surgery. However, it is important to choose a surgeon who is appropriately trained in fat transfer techniques to minimise the risk of complications. During your consultation, Dr Kian will explain the potential risks and complications of your procedure. Some of these include:

    • Scar – these are normally from small incisions to harvest (less than 1cm) and inject (few millimetres) the fat. They are usually placed discreetly but may be visible after surgery.
    • Infection – some infections may require antibiotic treatment.
    • Bleeding/haematoma – bleeding or a collection of blood that may result in swelling, firmness and scarring.
    • Seroma – this is a collection of fluid produced by the body.
    • Wound healing problems – this is unusual for the small incisions used.
    • Fat resorption/necrosis – there is usually some degree of fat resorption over time, however this varies between individuals. Fat that loses its blood supply can cause firm, tender lumps that may form scar tissue
    • Fat migration – this is when fat moves and settles down away from the site that it was originally placed.
    • Unfavourable results – this may be due to problems with asymmetry, under- or over-correction of your problem, contour/skin irregularities.
  7. How long is the recovery time?

    Recovery time is very individual, however for most people this will be between 7 to 14 days. For facial fat transfer, you will be asked to keep your head elevated for at least 48 hours after the procedure to keep the swelling down. We would normally advise to take a 2 weeks off work, however you may return to work sooner or later depending on how you recover and the type of work that you do. Full results take at least 6 weeks, however this will vary from person to person.

  8. What will the aftercare involve?

    Dr Kian believes that good aftercare is as important as the surgery itself. You will be given contact details to get in touch should you have any concerns after your surgery. You will have a clinic appointment approximately 1 week after your procedure. At this time, the wound is inspected to ensure that it is healing well and there is no infection. You will have a full review appointment with Dr Kian at approximately 6 weeks after your surgery to check that you are happy with your results and to give you long-term aftercare instructions.

You will have some small dressings over the incision sites. You will need to keep everything as clean and dry as possible for at least a week, when all the incisions have fully healed. You will be seen again for your review appointment with Dr Kian 6 weeks following your surgery.
